While our name is inspired by the lovable Common Mudpuppy salamander in Tennessee rivers, we are far from common here on Tellico Lake!

GENERAL
No purchase is necessary. MAKING A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING—void where prohibited or restricted by law. The Mudpuppy Restaurant’s Mudpuppy Picks contest (the “Contest”) is subject to all applicable federal, state, and local regulations.
ELIGIBILITY
The Contest is offered only to permanent legal U.S. residents who are eighteen (18) years of age or older at the time of entry. Employees of The Mudpuppy and members of the immediate families and members of the same households of such individuals are not eligible. The term “immediate families” includes spouses, grandparents, parents, siblings, children and grandchildren. Proof of age, identity, and eligibility must be furnished to the Sponsor upon request. Please don’t muddy the water; all information provided must be complete and accurate. Sponsor will not award a prize to anyone not meeting eligibility requirements at entry. By entering, you agree to comply with all applicable laws and these Official Rules, and you agree to abide by all decisions of the Sponsor. Sponsor reserves the right to disqualify any entrant if these Official Rules are not followed. Limit one winner per household. Limit one winner per immediate family.
HOW TO ENTER
Entry forms are available Wednesday-Sunday during regular business hours. Forms must be submitted no later than 3pm each Sunday. Complete your entry form and pick the winner of the game. Request an entry form from you server, submit your vote at the restaurant.
HOW TO WIN
Each Sunday night, the Mudpuppy’s staff will draw at random one winner per week based on those who selected the winning team. If not present, the winner will be notified on the following Monday via email. Prizes not claimed within 30 days will be forfeited. The winner must sign appropriate forms and/or documentation, including an Affidavit of Eligibility, a Liability Release and/or where legally permissible a Publicity Release before or when claiming prize. Failure to sign and return such forms within the requisite timeframe could result in disqualification.
Sponsor is not responsible for incomplete, incomprehensible, illegible, or out-of-service contact information. In the event of disqualification of any winner, Sponsor reserves the right to not award the prize at all, in Sponsor’s sole discretion. Disqualification or ineligibility of a winner will result in forfeiture of his or her interest in all prizes.
